COVID-19 SELF ASSESSMENT TOOL

To ensure the safety of all, we are re-opening on a slow scale currently operating at limited capacity.  This approach will continue to adapt based on the COVID restrictions and protocols in our community. We will be conducting COVID prescreening for everyone that would like to enter the ILA, this will speed up admission processes and is MANDATORY to enter the facility.  You can complete the self screening tool prior to coming to the Iroquois Lacrosse Arena.

Screening must be completed the same day that you are planning on visiting the ILA. And a copy must be available to show any staff member if required.
